How to make a claim for reimbursement.
If you have worked at Retama to raise funds for AFSA you are eligible to be reimbursed Selma food handlers certification, and Texas Alcoholic Beverage Commission (TABC) certification.
You will need to print out a copy of AFSA's Claim for Expenses
Form and either turn it in at one of our meetings or mail it to the address below. Don't forget to sign the form and include your return address and identify what you want to be reimbursed for.
